Terms of Service

Precision Weight Management ("we," "us") is a telehealth weight management practice operated at preciseweightmanagement.com. Clinical services are provided by a licensed nurse practitioner. We are not a pharmacy and do not dispense, stock, or ship medication.

You must be at least 18 years old, located in a state where our providers are licensed, and able to form a binding contract to use our services. By creating an account you confirm the information you provide is accurate and complete.

Content on this website is for general information only and is not medical advice. A provider-patient relationship is established only after a licensed provider reviews your intake and approves you for treatment. Submitting an intake form or paying the evaluation fee does not guarantee approval or a prescription.

If you are experiencing a medical emergency, call 911 or go to your nearest emergency room. Do not use this service for urgent or emergency care.

We charge a one-time, non-refundable $49 medical evaluation fee when you submit your intake. If approved, you will be enrolled in a monthly care program at the monthly fee communicated to you at approval (currently from $249/month for the Essential Care Program or from $299/month for the Advanced Care Program). Program fees cover clinical services only — provider evaluation, supervision, monitoring, and care coordination.

Any prescribed medication is dispensed, billed, and shipped separately by state-licensed 503A compounding pharmacies. Medication costs and shipping charges are not included in our program fees.

You authorize us to charge the payment method you provide for the evaluation fee and, upon approval, recurring monthly program fees. Your card is stored securely by our payment processor, Stripe. Monthly fees renew automatically until you cancel.

You may cancel your monthly program at any time from your account page. Cancellation takes effect at the end of your current billing period; no partial-month refunds are issued. The $49 evaluation fee is non-refundable in all cases, including if you are not approved. See our Refunds, Shipping & Cancellation Policy for full details.

Compounded medications, including compounded semaglutide and tirzepatide, are prepared by state-licensed 503A compounding pharmacies and are not reviewed by the FDA for safety, efficacy, or quality. Prescriptions are issued only when clinically appropriate, at the sole discretion of the treating provider.

You agree to provide accurate and complete health information, to keep your account credentials confidential, and to use the service only for your own care. Misrepresenting your identity or medical history may result in termination of services without refund.

To the fullest extent permitted by law, Precision Weight Management is not liable for indirect, incidental, or consequential damages arising from your use of the service. Our total liability for any claim is limited to the fees you paid us in the three months preceding the claim.

We may update these terms from time to time; continued use of the service after changes take effect constitutes acceptance. Questions may be sent to hello@preciseweightmanagment.com.
