Health information: medical history, current medications, allergies, weight and height, lifestyle habits, and related information you provide through our intake form.

Payment information: collected and processed directly by Stripe, our payment processor. We do not store your full card number on our servers.

Usage information: basic technical data such as browser type and pages visited, used to operate and improve the site.

We use your information to evaluate your eligibility for treatment, provide clinical services, process payments, communicate with you about your care, and comply with legal and regulatory obligations.

We share information only as needed to provide your care:

Pharmacy partners: prescription and relevant health details are transmitted to state-licensed 503A compounding pharmacies (including Strive and Stryker Pharmacy) to fill and ship prescribed medication.

Service providers: Stripe (payments), our hosting and database providers, and our electronic health record system (Charm EHR), each bound by their own privacy and security obligations.

Legal requirements: when required by law, regulation, or court order.

We do not sell your personal or health information.

Access to your information is restricted to authorized clinical and administrative personnel. Data is encrypted in transit, and medical records are maintained in a HIPAA-aligned electronic health record system. No method of transmission or storage is 100% secure, but we take reasonable measures to protect your information.

You may request access to, correction of, or a copy of your health information by contacting us at hello@preciseweightmanagment.com. You may close your account at any time; we retain medical records as required by applicable state law.
