HIPAA Notice of Privacy Practices

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

Our commitment to your privacy

Precision Weight Management provides telehealth weight management services. We are required by law to maintain the privacy and security of your protected health information (PHI), to give you this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect. We will let you know promptly if a breach occurs that may have compromised the privacy or security of your information.

How we may use and disclose your health information

The following categories describe different ways we may use and disclose your PHI without your written authorization:

For treatment. Your nurse practitioner and consulting pharmacy partners may use your health information to evaluate your condition, determine whether GLP-1 therapy is appropriate for you, prescribe medication, and coordinate your care. For example, we share your intake information and prescription with the dispensing pharmacy so they can fill and ship your medication.

For payment. We may use and disclose your health information to bill and collect payment from you or another party responsible for payment for the services we provide, including our monthly program fees.

For health care operations. We may use your health information to run our practice, review the quality of care we provide, train staff, and conduct business planning.

As required by law. We will disclose your health information when required to do so by federal, state, or local law, including in response to a court or administrative order, subpoena, or other lawful process.

Public health and safety. We may disclose health information for public health activities, to avert a serious threat to health or safety, or for other purposes permitted by the HIPAA Privacy Rule.

Uses and disclosures requiring your written authorization

Most uses and disclosures of psychotherapy notes, uses and disclosures for marketing purposes, and disclosures that constitute a sale of PHI require your written authorization. Other uses and disclosures not described in this notice will be made only with your written authorization. You may revoke an authorization at any time in writing, and revocation will not apply to actions we have already taken based on the authorization.

Your rights regarding your health information

  • Right to inspect and copy. You may request to inspect and obtain a copy of your health record. We may charge a reasonable, cost-based fee.
  • Right to amend. If you believe your record is inaccurate or incomplete, you may ask us to amend it. We may deny the request if, for example, it was not created by us or is already accurate.
  • Right to an accounting of disclosures. You may request a list of certain disclosures we made of your health information.
  • Right to request restrictions. You may ask us to limit the use or disclosure of your information. We are not required to agree, except that we must honor a request to restrict disclosure to a health plan for services you have paid for in full out of pocket.
  • Right to confidential communications. You may ask us to contact you at a specific address or phone number.
  • Right to a paper copy. You may request a paper copy of this notice at any time, even if you agreed to receive it electronically.

Security of your information

Your intake form, health history, and provider communications are transmitted over encrypted connections and stored on secure infrastructure. Only your care team and personnel who need the information to provide services may access your records.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services Office for Civil Rights. We will not retaliate against you for filing a complaint.

Changes to this notice
